Summary and QualificationsSummary:The Primary Physical Therapist in
Home Health manages an assigned caseload of patients, providing
therapy and clinical case management, taking responsibility to
improve patient outcomes and to manage resources and assure cost
effective, clinically effective, and coordinated services.
Completes comprehensive assessment/examination/evaluation,
develops/modifies plan of care in conjunction with
patient/caregiver and physician, provides treatment visits, and
provides discharge planning to these patients to facilitate
patient/caregiver engagement for achievement of patient goals,
clinical outcomes and positive patient experience. The Primary
Physical Therapist provides the majority of patient's physical
therapy and communicates handoff information/coordinates with PTA,
other therapists/members of care team to support patient's care
throughout home health and the continuum of care. -What
qualifications you will need:Required Education: Bachelor of
Science degree in Physical Therapy -Required License: Current North
Carolina license to practice Physical Therapy; Driver's license
required. BLS/CPR Certification -Required Experience: One to two
years related experience. -Preferred Experience: Clinical
affiliations having a wide variety of acute care procedures,
especially orthopedics, neurology, cardiopulmonary and care of
burns/open wounds are helpful. -
- Basic Cardiac Life Support must be obtained within 30 days of
employment start date
- Licensed Physical Therapy must be obtained within 2 years of
employment start date
- Bachelors Degree is a healthcare organization serving western
