Transitional Care Unit as a Registered Nurse and be part of a team
that provides specialized care to patients transitioning from acute
care to home or long-term care. In this role, you will use your
clinical expertise and compassionate approach to support patients
and their families during critical recovery periods. -Key
Responsibilities: -As an RN in the Transitional Care Unit, you will
assess and monitor patient conditions, implementing care plans and
interventions as needed. You will administer medications,
treatments, and therapies with precision and care. Collaborating
with a multidisciplinary team, you will coordinate patient care and
discharge planning. Educating patients and families on
post-discharge care and self-management will also be a crucial part
of your role. You will maintain accurate documentation and adhere
to hospital policies and procedures. Additionally, you will

expertise!Job Summary and QualificationsProvides leadership which
facilitates the delivery of professional nursing care for the
comfort and well-being of patients. Utilizes the nursing process of
assessment, planning, implementation, and evaluation, to provide,
delegate, supervise and document care and teaching of patients and
family. Implements medically prescribed interventions.What you will
do in this role:
- Assess the patient's condition during admission and each
scheduled shift, promptly identifying and reporting any changes in
patient status.
- Perform procedures, monitoring, or other functions as ordered
by the medical provider(s), and ensure thorough and timely
documentation of care administration in the patient's medical
record. - Administer prescribed medications, monitor the patient
for therapeutic response, and take appropriate action in the event
of an unintended response to the medication.
- Provide exceptional care by responding promptly to patient
requests, proactively anticipating patient needs, and resolving
them.
- Educate patients, families, and caregivers about the patient's
medical condition, treatment plan, medications, possible side
effects, and follow-up measures, ensuring complete understanding by
translating complex medical terminology.What qualifications you
will need:
- Preferred Experience: -One year hospital nursing
experience
- Required Education: -Bachelor of Science in Nursing or enroll
in a BSN or advanced degree program and obtain a BSN within 5 years
of hire date into RN position. Grandfathering will be accepted
between departments and Mission Health entities. Exemptions, or
extensions, may be approved by the facility CNO and reported out to
the CNO Council members. Each facility will ensure tracking of
exemptions should this information be needed. RNs with 10 years or
more of experience are exempt.
- Required License/Certifications: -Must have and maintain
current licensure as registered nurse with the North Carolina Board
of Nursing. eNLC (Enhanced Nurse License Compact) license may
apply, licensee should confirm with NCBON
- Certificate: -BLS Healthcare Provider is a full-service
