business needsLocationThis position will travel to see home/nursing
home/assisted living patients in the McDowell county areaPosition
Summary: Coordinates the nursing plan of care, provides direct
nursing care, and instructs/supervises the LPN and Hospice Aide
staff in the delivery of nursing care to clients.Manages the
nursing care for an assigned caseload of approximately 15 patients
with varying acuity and needs. Caseloads above this target will be
co-managed or additional support will be offered. In the event the
assigned caseload falls below this target, the nurse may be asked
to provide support to other team members.Essential Duties &
Responsibilities:Practice within the scope of license,
certification or registration, where applicable, and as required by
law and regulationAssess the patient's physical and mental health
including the patient's reaction to illnesses and treatment
regimens.Record and report the results of the nursing
assessment.Plan, initiate, deliver, and evaluate appropriate
nursing acts.Teach, assign, or supervise other personnel in
implementing the treatment regimen.Collaborate with other health
care providers in determine the appropriate patient and
family-centered plan.Implement the treatment and pharmaceutical
regimen prescribed by any person authorized by State law to
prescribe the regimen.Provide teaching and counseling about the
patient's health.Report and record the plan for care, nursing care
given, and the patient's response to that care.Supervise, teach,
and evaluate those who perform nursing functions and administer
nursing programs and nursing services.Provide for the maintenance
of safe and effective nursing care, whether rendered directly or
indirectly.Knowledge, Skills & Abilities:Must be willing and able
to care for patients with infectious disease processes unless
approved accommodationsKnowledge of Federal, State and Local
regulations and Medicare Conditions of ParticipationKnowledge of
HIPAA privacy rules and regulationsKnowledge of electronic medical

Qualifications:Experience:Two years of clinical experience
