Job Description:
A company is looking for a Transition of Care Specialist -
Remote (Must live in Hawaii).
Key Responsibilities
Manage Transitions of Care protocols for members in inpatient or
residential settings
Coordinate post-discharge follow-up appointments and ensure
connection with healthcare providers
Maintain knowledge of community resources and assist in discharge
planning for hospitalized patients
Required Qualifications, Training, and Education
2 years of experience in psychiatric or substance use behavioral
health settings related to discharge planning
Experience managing a caseload of low complexity care management
members
Applicable experience in Medicare, Medicaid, or Whole Health
markets
Associate degree in Nursing or Bachelor's degree in Social Work
Understanding of psychiatric/medical terminology and call center
operations
